Membership

At Foundation for Disease Awareness and Prevention (FDAP), we believe that meaningful change is possible through collective action. We welcome individuals who share our mission, vision, and commitment to advancing disease awareness, prevention, and public health. Our members play a vital role in supporting initiatives that create healthier communities across Bangladesh. 

1.Honorary Membership

Eligibility Criteria

  • MBBS/BDS graduates with valid BMDC registration/license.
  • Health professionals with a Master of Public Health (MPH) or an equivalent postgraduate degree in a health-related field.
  • Minimum two (2) years of relevant professional experience.
  • Demonstrated commitment to FDAP’s mission, vision, and objectives.

Willingness to contribute expertise and actively support health awareness, disease prevention, and community initiatives.

Membership Fee & Validity

  • Membership Fee: BDT 10,000 (Ten Thousand Taka)
  • Membership Validity: Five (5) Years

Termination of Honorary Membership

Honorary Membership may be terminated if a member:

• Violates FDAP’s rules, policies, or code of conduct.
• Misuses FDAP’s name, logo, or position.
• Engages in serious professional misconduct or unlawful activities.
• Provides false information or acts against FDAP’s interests.
FDAP reserves the right to suspend or terminate membership following a fair review.

2.Life Membership

Eligibility Criteria

  • Open to individuals who support and share FDAP’s mission, vision, and objectives.
  • Minimum 18 years of age.
  • Demonstrated interest in health awareness, disease prevention, community well-being, or social development.
  • Willingness to support and participate in FDAP’s programs, campaigns, and community initiatives.
  • Commitment to uphold FDAP’s rules, policies, and code of conduct.

Membership Fee & Validity

  • Membership Fee: BDT 5000 (Five Thousand Taka)
  • Membership Validity: Lifetime

Termination of Life Membership

Life Membership may be terminated if a member:

  • Violates FDAP’s rules, policies, or code of conduct.
  • Misuses FDAP’s name, logo, or membership status.
  • Engages in serious misconduct or unlawful activities.
  • Provides false information or acts against FDAP’s interests.

FDAP reserves the right to suspend or terminate membership following a fair review.
